Completing and Submitting the Application.
- Please use the ADA internship recommendation form. The internship application must be completed in detail.

- Along with the internship application, please include a cover letter that describes your interest in becoming a registered dietitian and your career goals. Please do not exceed two pages.
- Secure and enclose a signed Verification of Completion or Intent to Complete form from your Didactic Program Director. If accepted into the program and you sent an Intent to Complete form with your application, you must obtain a Verification of Completion and an official final transcript with your degree recorded prior to the start of the internship.
- Secure official copies of all transcripts in sealed envelopes and include in the application packet.
- Submit in sealed, signed envelopes at least three completed recommendation forms. A sample form is included in your application packet. You will need to make additional copies for those individuals you have selected as references. These letters should include, where possible, a letter from the Didactic Program Director at the institution where the Didactic Program requirements were completed and verified, a professor or instructor from your Didactic Program, and one other person with knowledge regarding your suitability for a Dietetic Internship Program

Materials must be received by the deadline above. Late materials and incomplete packets will not be reviewed.
Applicant must meet appropriate recency requirements. Verification statements older than 5 years may not be considered.

Evaluation of Interns
Interns are evaluated regularly and frequently during their six-month rotation. Informal evaluations take place in discussions between the DI Director and Program Preceptors on a regular basis. Formal, written evaluations will be competed at the end of each rotation, with input from the intern. Interns will be evaluated based on the specified competencies considered essential for an entry-level dietitian set by the CADE. Professional behavior and attitude will also be evaluated and documented in the evaluation form. Another evaluation will be completed by each intern to provide the DI Director with input as to the effectiveness of the program.
In the event of unsatisfactory performance during any of the rotations, the preceptor and DI Director consult to determine the format of a redirection plan to allow the intern to achieve the competencies. Inability to accomplish that plan may result in dismissal from the program.

In order to consider your application, the following items must be received in our office on or before the deadline:
- A $50 non-refundable application fee. See above for instructions.

- Verification statement or Intent to Complete form
- Transcripts
- Letters of References (provided on the ADA recommendation forms)
- NOTE: Send your D & D Digital Application directly to them by the deadline specified in their brochure.
